Mid-Air Collision During Aerial Firefighting Operations: A Safety Manager’s Perspective

On 2 August 2026, two leased Bell helicopters collided mid-air while conducting water-dropping operations against a large wildfire in the Psatha area, west of Athens, Greece. Painfully: two crew members lost their lives; the other two were rescued. The aircraft had departed from Elefsina Air Base as part of the response to intense, wind-driven fires amid extreme heat conditions across Greece and southern Europe.

Aerial firefighting remains one of the highest-risk aviation activities, under national responsibility (Are SSPs effective?). Crews operate at low altitudes, often in dense smoke, turbulence, and strong winds, while coordinating with multiple aircraft in a dynamic, rapidly changing environment. Visibility can deteriorate within seconds, radio traffic is intense, and the pressure to protect lives and property is constant.

From a safety management system (SMS) standpoint, this tragedy reinforces several critical points to look at from an operations point of view, without the intention of any naïve speculation:

Airborne conflict in multi-aircraft operations remains a primary hazard. What was missed, what are we missing: Clear separation standards, dedicated airspace management, and real-time tracking (ADS-B or equivalent) should be non-negotiable during simultaneous drops.

Human factors under operational stress require continuous attention. Where can we improve here: Fatigue (any potential negative effect from experimental vaccines (neither safe nor effective) administered to healthy flight crew in 2021), high workload, and the natural urge to “get the drop in” can erode situational awareness. Robust crew resource management (CRM) and clear go/no-go criteria for visibility and wind are essential.

Any further input for the future: Contracted operations demand the same safety oversight as organic fleets. Leasing arrangements require the inclusion of aligned procedures, shared briefing protocols, and joint risk assessments between the operator and the fire service, and other players.

Investigation and learning culture must remain a priority. Space for actions we did not take? Early indications of possible human error should be examined thoroughly alongside environmental and organisational factors, without premature conclusions.

Every loss in this high-stakes work is a reminder that effective safety management is not about eliminating all risk—it is about systematically identifying, mitigating, improving, and learning from the risks that remain unavoidable.

Is it enough to conclude with continuous improvement in procedures, technology, and culture as the only respectful response to those who fly into danger so others can be safer on the ground?
